- Quality Improvement: Lead and participate in continuous quality improvement activities aligned with the company’s Quality Management System.
- Client Care: Ensure all clients receive holistic assessments and ongoing reassessments to promote positive client and clinical outcomes.
- Care Planning: Develop inter-professional, client-centered, goal-oriented care plans with clients and families to ensure high-quality, safe care and positive health outcomes.
- Client Visits: Manage care and services by visiting clients as needed, ensuring satisfaction, safety, and positive outcomes.
- Team Engagement: Promote and improve team engagement and client experiences through various methods.
- Performance Management: Provide constructive, timely, and ongoing feedback and performance management.
- Learning Environment: Facilitate a learning environment by translating evidence-based knowledge into education to optimize patient outcomes and experiences.
- Education Participation: Attend and participate in internal and external education offerings, ensuring knowledge dissemination to team members.
- Mentorship: Provide mentorship to paraprofessional staff and foster the company’s mentorship framework within the team.
- Client Satisfaction: Lead or assist with client satisfaction initiatives, addressing and documenting client concerns and complaints promptly.
- Best Practices: Demonstrate knowledge of current best practices and standards, liaising with regulatory bodies as needed.
- Interdisciplinary Liaison: Liaise with funders, government programs, case managers, and other interdisciplinary care team members.
- Human Resources: Participate in recruitment, selection, orientation, training, discipline, and performance reviews of direct reports.
- Task Delegation: Implement, monitor, and evaluate delegated tasks to unregulated care providers.
- Business Development: Participate in business development and assist with developing contractual agreements for client care and service
- Company Promotion: Promote the company through participation in local healthcare or business committees and community events.
- Electronic Health Records: Advance the effective use of electronic health records and use data to improve clinical outcomes and practice standards.
- 24/7 Access: Ensure 24/7 access to a clinician for employees and clients, serving as the on-call nurse/clinician as required.
- Professional Development: Participate in ongoing professional development and continuing education activities, providing mentorship to other supervisors and administrative team members.
- Graduation from an accredited Licensed/Registered Practical Nursing Program;
- Current registration with the provincial regulatory body.
- Additional training and/or certifications such as CNA certifications, Learning Essentials for Advanced Palliative Care (LEAP) or dementia training would be an asset to this role;
- Competencies will be dependent on funder and branch requirements.
- At least five years of recent experience with progressive responsibilities, as a Home Support Supervisor, preferably in a community setting;
- Strong track record in leading the development and evaluation of training programs;
- Demonstrated ability to handle all aspects of human resources and interpret company policies and procedures;
- Prior experience in business development preferred.
- Proven computer technology skills e.g. Microsoft applications, and ability to use virtual meeting forums.
- Solid understanding of the principles, practices, and methods of community nursing and service delivery
- Exceptional interpersonal skills and ability to handle difficult situations objectively and consistently;
- Ability to work independently and effectively lead a high performing team;
- Demonstrated team-building skills;
- Excellent writing, reporting and English communication skills;
- French language proficiency considered a strong asset
- Valid driver’s license and auto insurance
- Evidence of current CPR certification; and clear criminal background check including vulnerable sector screening
