Accommodations are available upon request for candidates participating in the recruitment and selection process.Summary of responsibilitiesReporting to the Manager of Chronic Disease Management, the Coordinator provides operational support to the manager and staff of the Chronic Disease Management outpatient clinics. The incumbent's leadership fosters a culture of safety and quality. They adopt a collaborative approach, demonstrating a strong team-oriented mindset and a commitment to creating a positive and supportive work environment. The Coordinator assists in overseeing operations and quality improvement initiatives aimed at continuously enhancing employee engagement, productivity, and organizational performance within the Chronic Disease Management portfolio, ensuring alignment with corporate and regional goals. Additionally, they respond to daily inquiries related to staff scheduling, office space and associated resources. The Coordinator manages and monitors all data entry in UKG (schedules and payroll) and contributes to the development and evaluation of both short- and long-term goals and objectives for the program, along with other required quality initiatives.Requirements of the job- Undergraduate Degree or diploma in Science or Healthcare related discipline
- A minimum of 3 years of relevant experience in outpatient clinic environments
- Customer service-minded and a results-focused, can-do attitude
- Strong critical thinking, problem-solving, and conflict resolution skills
- Ability to prioritize, respond to demanding workloads, adapt to changing priorities, and multitask effectively.
- Exceptional attention to detail and high level of accuracy
- Ability to exercise sound judgment, discretion, tact and maintain a high degree of confidentiality
- Proven ability to work effectively with all members of the health care team
- Operational knowledge of CCH policies and collective agreements
- Proficient in navigating Electronic Health Record and MS Office software (Word, Excel)
- Demonstrated regular, punctual attendance
- Commitment to patient safety
- Ability to demonstrate the ICARE values in their practice
- Functional level in both official languages (oral and written in English; oral in French, and testing completed)Asset:
