Clinical Practice Manager – National Role
Location: HybridAre you a Registered Nurse (RN) or Registered Practical Nurse (RPN) with strong experience in long-term care and a passion for clinical excellence, education, and quality improvement?This is a unique opportunity to step into a national-level role , where you will influence clinical practice, support teams across multiple homes, and help shape how care is delivered at scale.Reporting to the Senior Director, Quality & Business Enablement, this role is responsible for leading, developing, and supporting clinical programs across a national portfolio of long-term care homes .Key Responsibilities
  • Provide national oversight of clinical programs , ensuring alignment with provincial regulations, Accreditation Canada standards, and best practices
  • Lead the development and continuous improvement of clinical policies, procedures, tools, and education resources
  • Support homes with implementation, training, and ongoing clinical guidance
  • Act as a clinical subject matter expert , partnering with Operations, Medical, Compliance, Risk, and Redevelopment teams
  • Serve as a PointClickCare expert , supporting clinical workflows, documentation standards, and system optimization
  • Lead Accreditation Canada readiness , including education and survey preparation
  • Facilitate education sessions and communities of practice for clinical leaders and teams
  • Support Regional and home-level leaders in building clinical competency, accountability, and program consistency
  • Conduct home visits to assess implementation and support continuous improvement
  • Partner with vendors to ensure consistent service delivery, training, and support across homes
  • Support development and monitoring of clinical KPIs, audits, and performance metrics
  • Provide guidance and escalation support when homes are not meeting clinical standards or targets
What You Bring
  • RN or RPN designation
  • Strong experience in long-term care or seniors living environments
  • Solid understanding of LTC legislation, compliance standards, and clinical documentation requirements
  • Experience with PointClickCare, RAI-MDS 2.0 , and familiarity with interRAI
  • Passion for education, coaching, and supporting clinical teams
  • Proven ability to lead initiatives, manage projects, and drive change
  • Strong communication and relationship-building skills
  • Ability to work effectively in a fast-paced, evolving environment
