- Promotes the development and maintenance of patient/family focused care, taking direct responsibility for effective coordination of care outcomes and communicates with patient and families accordingly
- Facilitate behavioral management care plans as needed
- Creates an environment that maximizes participation in decision making within the team.
- Acts as a consultant and expert in coordinating daily patient care assignments
- Attends, facilitates, and/or participates in unit, hospital-wide, and community-based meetings/committees as invited (e.g. Medicine Operations, Medicine Leadership Team meetings, Unit Based Council, daily Patient Flow meetings)
- Facilitates a positive learning environment and acts as a resource for staff and health professional students, including onboarding new staff and coordinating placements
- Develops and maintains effective linkages with peers, hospital departments and programs
- Participates in the development, implementation and review of hospital programs, policies and procedures as required
- Actively participate in and support endeavors to enhance quality care
- Assist Patient Care Manager in maintaining professional Nursing Standards and Competencies.
- Assists in the implementation and evaluation of targets, goals and objectives as identified both internally and externally
- Provides direction and guidance to all members of the health care team, patients and their families, groups and organizations and collaborates with the health care team members to plan for the care of patients and families.
- Identifies patient care system problems and makes recommendations for change
- Assists with all Quality-Based Procedures (QBPs) within the medicine program; ensuring pathways are followed
- Awareness of Key Performance Indicators (KPIs) performance and proficiency
- Review incident report if delegated, and responds to patient complaints
- Other duties as delegated by PCM
- Monitors resources and utilization in partnership with the PCM
- Responds to changes in the team, ensuring that adequate staff coverage is available across the teams including provide direct assistance with patient care as needed in relief
- Responsible for all daily staffing including assignments, replacement of sick calls and monitoring all schedules to ensure budgetary objectives and compliance with the Collective Agreements
- Review organizational status board for surge status and escalation process.
- Review EDDs (Expected Date of Discharge) and level of care in Meditech before bed meeting and end of day, if applicable
- Prioritize reviewing discharge plan for EDDs of that day and follow up missed EDD
- Ensure every patient has a current and accurate EDD and plan of care within Meditech
- Identify barriers for discharge and coordinate pending diagnostics and to expedite discharge
- Collaborate with Patient Flow Coordinator to discuss admission, discharge, and transfer.
- Collaborate with Charge Nurses and Physicians to ensure effective and efficient Patient Rounds
- Create capacity for patients requiring private room due to isolation, medical necessity, or preferred accommodation
- Possesses a valid Certificate of Competence from the College of Nurses of Ontario.
- Baccalaureate in Nursing, Health Studies, or Administration.
- At least 5 years acute care or ICU
- 3-5 years leadership experience (Charge Nurse or similar experience)
- 2-3 years supervisory experience preferred
- Certification in oncology, Coronary Care 1 and 2 preferred
- RNAO member preferred
- Leadership and teaching/adult education experience preferred
- Masters preferred
- Excellent verbal and written communication and teaching skills
- Minimum 5 years of current acute and or critical care experience required
- Highly developed critical thinking and decision making skills
- Ability to manage conflict resolution and negotiation at all levels of the organization
- Excellent leadership skills and the ability to coach, and motivate the health care team
- Self-directed and able to deal with multiple stakeholders while managing multiple priorities.
