- Lead the delivery of holistic care by collaborating with nursing, personal support workers, allied health professionals, and social workers.
- Partner with hospitals, Home and Community Care Support Services (HCCSS), and other community organizations to ensure safe and effective patient transitions.
- Provide mentorship, supervision, and performance management to personal support workers and nurses within your program.
- Ensure all services meet federal and provincial standards, as well as internal policies and best practices.
- Conduct case reviews and monitor client outcomes to ensure care goals are being met efficiently and effectively.
- Design and implement individualized care plans in collaboration with hospital partners to support safe discharges and continuity of care.
- Address patient and family concerns with professionalism and empathy, ensuring timely follow-up and resolution.
- Support staff development through home visits focused on training, performance evaluation, and skill-building.
- Analyze intake processes, priority coding, and service refusals, and implement strategies to improve access and efficiency.
- Oversee the completion of client assessments and ensure documentation is thorough and accurate.
- Collaborate with the Clinical Manager to track program metrics and communicate outcomes to the data management team.
- Escalate care or service-level concerns to the Clinical Manager for further review and resolution.
- Foster strong internal and external relationships through clear, compassionate communication.
- Participate in case conferences, team meetings, and ongoing professional development activities.
- Be available for flexible scheduling, including evenings and weekends, and participate in the on-call rotation as needed.
- Minimum 5 years in healthcare, preferably in home care and hospital settings.
- Registered Practical Nurse (Ontario), graduate of an accredited program, current CPR certification.
- Strong understanding of community nursing, service delivery, discharge planning, and Ontario’s health system.
- Proven ability to mentor and lead teams with compassion and consistency.
- Familiarity with RAI tools is an asset.
- Valid driver’s license and reliable vehicle.
