
Social Service Worker, Temporary Full Time, Access to Care & Transitions

- Social Service Work diploma with active, unrestricted license to practice with the College of Social Workers and Social Service Workers of Ontario
- One year experience in navigation, transition and discharge planning activities in a hospital setting preferred
- Conducts patient assessment including identification of baseline status, family assessment, reports by other service providers etc, assuming responsibility for service coordination for referred patients
- Participates in the development of individualized patient care transition plans in collaboration with other team members, external partners, the patient and family
- Ability to perform comprehensive social assessments, monitor and evaluate patient progress and revise plans as required
- Identifies and prioritizes patient supports and linkages required in support of timely discharge referring to community supports and services for continuing care
- Demonstrates leadership in the practice setting by acting as a role model and resource of the interprofessional team in support of navigation, transition and discharge planning activities and specifically social service expertise
- Specialized sub-acute Care in rehabilitation, mental health services, complex medical management and palliative care
