- Review medication orders, patient profile, and treatment regimen within Meditech using Oncology module for clinical appropriateness, distribution effectiveness, and billing accuracy
- Ensure safe, timely and efficient processes for compounding, and dispensing of medication used for Cancer Clinic, Medical Day Care and inpatient needs
- Support distribution activities (e.g., staging, checking, etc.) together with pharmacy technicians as required
- Maintain accurate documentation of administrative related tasks (e.g., eClaims, etc.) for reporting (e.g., financial)
- Timely documentation of patient assessments, medication reconciliations, clinical interventions and pertinent therapeutic notes within the patient electronic record
- Support maintenance of adequate inventory levels, and minimizing drug wastage together with pharmacy technicians.
- Work collaboratively with other pharmacy staff and health care providers to develop institutional guidelines and make evidence-based decisions designed to improve patient care, enhance safety, and improve quality of care.
- Participate on committees (including at regional level) to develop policies and implement programs to ensure the safety of staff and patients during the receipt, preparation, administration, and monitoring of anticancer agents.
- Maintain up-to-date awareness of current CCO-related communications (e.g., treatment funding), latest evidence based research, and best practice recommendations
- Participate in routine system and process maintenance based upon relevant policy, workflow requirements, and best practice recommendations
- Support pharmacy departmental objectives and act as clinical subject matter expert for other pharmacy staff
- Support Drug Access Facilitator (DAF) clinical and administrative duties as required
- Provide clinical and drug distribution support to inpatient oncology unit as required
- Assist with troubleshooting of clinical, distributive, billing, and system issues as required
- Occasionally provides back-up pharmacy service coverage of inpatient units or departments
- Other duties as assigned.
- B.Sc. Phm required
- Licensed under Part A of the Ontario College of Pharmacists register required
- Hospital pharmacy residency or equivalent acute care clinical experience preferred
- Demonstrated clinical pharmacy experience in an acute care hospital setting preferred
- Proficiency with CPOE (Computerized Physician Order Entry) system (e.g., Meditech Expanse or EPIC) preferred
- Advanced knowledge of medication therapy management and clinical decision-making
- Competency in medication order review, clinical assessment, and therapeutic recommendations
- Aseptic technique and dispensing skills
- Demonstrated high degree of accuracy and attention to detail
- Excellent clinical problem recognition and problem-solving skills
- Proficient in the use of Microsoft Office programs
- Able to manage multiple clinical responsibilities with competing priorities in a fast-paced acute care environment
- Well-organized, self-directed, able to set priorities and manage time effectively
- Able to interact patiently, effectively, and professionally with interdisciplinary healthcare teams, physicians, and patients
- Committed to clinical excellence, patient safety, and evidence-based practice
- Excellent verbal and written communication skills
- Able to meet the physical demands of the position
