(employee will be working with or have access to children or vulnerable adults and requires a Criminal Record Check)Required for Professional RegistrationJob DescriptionIn accordance with the Mission, Vision and Values, and strategic directions of Vancouver Island Health Authority (Island Health) patient safety is a priority and a responsibility shared by everyone; as such, the requirement to continuously improve quality and safety is inherent in all aspects of this position.Working collaboratively and without general supervision, and as a member of an interdisciplinary and practice linked health care team, the Social Worker delivers culturally safe, collaborative and integrated care by providing assessments, care planning and coordination, intervention, consultation, and referrals for services to improve health and care of patients, including the frail elderly, those with chronic/complex diseases, those with mental health and substance use challenges, and vulnerable children and youth, and reduce demand on acute services where the social, economic, emotional or psychological circumstances of the patient presents barriers to care and/or an optimum level of function.QUALIFICATIONS:Education, Training And ExperienceBachelor's Degree in Social Work from an accredited university.Current registration with the British Columbia College of Social Workers (BCCSW).Demonstrated knowledge of applicable legislation and available programs and services, their interrelationships and the function in delivering care in the community; knowledge of the community demographic and issues, and remote community dynamics; or an equivalent combination of education, training and experience.Valid BC driver's license.Skills And Abilities
