Reports to: Brain Allied Health Manager
Hours: 37.5hrs, Mon-Fri
Status: Temporary Full -Time, 6 months
Closing Date: September 2, 2025POSITION: In the provision of patient and non-patient care activities, the primary responsibilities of the Social Worker include: innovative application of social work processes that assist patients with psychosocial challenges and to optimize client progress in rehabilitation through the continuum; providing comprehensive psychosocial assessments and intervention; providing educational and supportive counselling services; providing service based on individual, family and group practice models; discharging planning, population health advocacy, community services linkage; collaborating with the inter-professional team and community-based service providers; advocating, mediating and resolving conflict; participating in professional, program and hospital development; willingness to provide field practicum Instruction to Social Work students; participating in current and future program development, quality initiatives and research activities; complying with, and commitment to, patient safety responsibilities and corresponding hospital plans, policies and procedures in order to ensure a safe environment for patients.Qualifications
- Completion of a Master's Degree in Social Work (MSW) or equivalent required
- Registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) required
- Membership with Ontario Association of Social Workers (OASW) preferred
- Two years experience working with a interdisciplinary team in a health care setting preferred
- Experience with with ABI and stroke patient populations preferred
- Excellent verbal and written communication skills
- Excellent interpersonal, time management and organizational skills
- Excellent problem-solving skills
- Demonstrated ability to integrate theoretical concepts and research findings into practice and maintain an evidence-based, best practice approach
- Demonstrated ability to supervise students within the context of a teaching hospital
- Ability to provide a blend of adjustment counselling and instrumental interventions
- Working knowledge of objective, self-assessment, single system assessment and evaluation is desirable
- Proven ability to work independently and as part of an interprofessional team
- Demonstrated ability to identify and meet own learning needs in pursuit of ongoing professional development
- Demonstrated ability to provide CBT (Cognitive Behavioural Therapy) in an individual and group format preferred
- Demonstrated ability to do group work preferred
- Computer literacy preferred
