- Assist clients in preparing for health appointments and meeting daily needs
- Provide nutritional guidance and ensure food security (non-perishable food stocks)
- Check home environments for safety, risk, and cleanliness
- Support clients with mobility access and equipment
- Promote and advise on healthy living and community involvement
- Help plan and organize recreational and social activities
- Collaborate with client families on planning and care
- Attend and support client planning meetings
- Maintain clear and organized reports and documentation
- Perform other duties as assigned
- This role requires work in client homes and may involve shift work.
- Legal eligibility to work and reside in Canada.
- Work independently and collaboratively
- Remain calm and compassionate in high-pressure or emergency situations
- Respect cultural and individual differences
- Be physically capable of lifting/moving up to 40 lbs. and navigating stairs
- Be COVID-19 vaccinated (minimum two doses)
- Possess a valid driver's license and access to a reliable vehicle
- Clear an RCMP Vulnerable Sector Check
