The Sunnybrook Academic Family Health Team seeks a certified Physician Assistant (PA) to join its well established Family Health Team. The PA will be involved in the provision of clinical care, participation in quality and safety processes and involvement in trainee education.Responsibilities:While working with a team of health professionals in an inter-professional collaborative primary care setting and under the supervision of a physician, the role of the PA includes:Triage incoming patient calls: Assess patient health care needs, discuss test results and respond to patient questions pertaining to health promotion and disease prevention.For in person visits: conduct patient interviews, take medical histories, perform physical examinations and interpret findingsAssist with outreach in the management of chronic diseaseCounsel patients on preventive healthcare and chronic disease managementOrganize follow-up and same day appointmentsManage, assess and organize primary health care deliveryParticipate and contribute to Inter-professional team meetings, case conferencesPerforming certain acts under medical directives delegated by a physicianMaintaining accurate, concise and confidential documentation pertaining to patient history and encounter data.Performing other duties within your competencies as a physician assistant.Job RequirementsSuccessful completion of the Physician Assistant Bachelor Degree from an accredited educational institution.Certification of Physician Assistants (NCCPA).Current BCLS and CPR certification preferred.Previous experience using Electronic Medical Records (EMRs) – TELUS PSS preferred.Knowledge and proficiency in current, evidence-based methods and practices of primary care delivery.Demonstrate good problem solving and analytical skills.Demonstrate strong communication and interpersonal skills.Sound understanding of and commitment to patient and family-centered care.Ability to exercise sound judgment and discretion, make decisions and take initiative while working with confidential patient information and responding to patient inquiries.Superior work ethic, professionalism, and reliability; demonstrated commitment to providing outstanding patient care.Superior written/oral communication skills; excellent interpersonal and relationship building skills; ability toeffectively convey factual information and detailed explanations.
