Your goal in this position is to assist nursing personnel in providing personal care to residents and perform such procedures and treatments for which you have received special instructions. You will also perform other duties associated with the activities of daily living of the residents. This position supports the Hospital's academic mandate by advancing care through teaching and learning and/or transforming care through research and innovation. This position contributes to the shared responsibility of ensuring health and safety policies are followed to provide for a healthy environment for staff, physicians, volunteers, learners, patients and families.On a practical level, you will
- Perform all duties in accordance with legislation, the Mission Statement, Organizational Philosophy and Administrative policies and procedures of North York General Hospital Reactivation Care Centre
- Perform direct and indirect patient care activities in order to carry out the Interprofessional Care Plan. The scope of activities are directed and assigned by the RN/RPN or appropriate multidisciplinary team member.
- Under direction from a Registered Health Care Provider attends to the personal care needs of the patients including but not limited to: hygiene, grooming and dressing, toileting, recording nutrition and hydration intake, provision of preventative skin care
- Supports patients' independence, dignity, social, emotional and cultural well-being
- Work is moderately heavy involving moderately heavy lifting, walking, standing, bending, pushing and pulling of equipment
- Communicate effectively with elderly people and colleagues
- Registered and in good standing with The Health and Supportive care Providers Oversight Authority (HSCPOA)
- Completed Personal Support Worker (PSW) certificate from a College of Applied Arts and Technology
- 1-2 years experience in inpatient Geriatric/Rehab or Long-Term Care experience
- Experience working with patients with cognitive impairment/dementia and experience working with patients who exhibit responsive behaviors preferred
- Demonstrated Knowledge of Infection Control Practices
- Demonstrated aptitude to foster caring and trusting relationships with elderly patients and families
- Demonstrated ability to work well with team members
- Effective organizational and interpersonal communication skills
- Ability to perform responsibilities in a courteous manner
- Ability to function independently as well as part of a multidisciplinary team in a fast paced environment
- Commitment to ongoing education and development
- Demonstrated excellent customer service, organizational and problem solving skills (test will be administered)
- Excellent Attendance Record
- Skilled in Point of Care (POC), preferred
- Ability to read, write and communicate in English required
- Ability to speak a second language an asset
- Bilingualism in French/English an asset
