- Respond to continuing care center and health center’s calls requesting nursing assessments
- work within the community to develop and modify client care plans based on the assessed needs of the client and their prescribed medical care plans
- Work within the scope of practice providing healthcare services to adult clients for continuing care- including wound care, chronic disease management, medication administration and conducting health program requirements
- Consult with Physicians/Nurse Practitioners and the Continuing Care Center and/or Health Center Supervisor of Health Programs
- Utilize the skills of observation, assessment, nursing diagnosis, counseling, and health teaching according to care plan
- Ensure that, on an ongoing basis, the emotional, spiritual, physical comfort and safety of clients are met to the greatest possible extent within the scope of practice
- HCN assignments in Nunavut vary from 4 to 6 weeks minimum

- You are a graduate from an accredited School of Nursing and have a nursing license through the provincial governing body in good standing
- You have or are willing to obtain a nursing license through the College and Association of Nurses of the NWT and Nunavut (CANNN)
- You have one (1) year of recent experience (total 1500 hours) working in isolated, semi-isolated, or remote communities or two (2) years of experience (total 3000 hours) performing continuing care, home & community care nursing activities in home & community and/or Continuing Care Centers & Long-Term care setting
- You are willing to adhere to the policies and procedures under the direction of the Government of Nunavut
- Mandatory skillsets and additional training requirements and reimbursements will be discussed during the interview
