- Bachelor of Science in Pharmacy, or PharmD
- Current, unrestricted registration with the Ontario College of Pharmacists
- Successful completion of a Pharmacy Residency Program accredited by the Canadian Hospital Pharmacy Residency Board, preferred
- Minimum of three years of experience in hospital pharmacy practice, preferred
- Perform therapeutic review of client medication orders and prescriptions to ensure completeness, accuracy and appropriateness
- Identify and communicate actual and potential drug related problems to pharmacist or physicians as appropriate
- Function as a clinical pharmacist in the provision of direct client care
- Identify, resolve and prevent drug-related problems using a client-centered, collaborative approach
- Individualize medication regimens using sound principles, accounting for pharmacodynamics and pharmacokinetic variations in drug absorption, distribution, metabolism and elimination; interpret serum drug levels and make recommendations
- Specialized sub-acute Care in rehabilitation, mental health services, complex medical management and palliative care
