Apply today to join our team!As a Social Work Assistant with VCH you will:
  • Work under the general operational supervision of the Patient Services Manager, clinical supervision from the Practice Coordinator and receive work direction from Social Workers in a team based environment and provides social work support services during the admission and discharge of patients/clients.
  • Perform duties including interviewing clients/families/facilities for information, assisting clients to approach external resources, providing practical support to clients regarding benefits, housing, transportation, community resources, finances, and acquiring and maintaining a supply of up to date information and applications on services/agencies/resources.
About Adanac Park Lodge (APL):Adanac Park Lodge (APL) is a regional Vancouver Coastal Health (VCH) Long Term Care (LTC) program specializing in providing care to younger adults (19-65 years) with complex health and behavioral needs. APL’s unique population of focus is younger adults who have concurrent Mental Health, Substance Use and/or Acquired Brain Injury; and who often have co-existing physical, developmental and/or chronic health conditions. Clients who may be considered for admission are between19 and 65 year of age. Residents may reside at APL for as long as they require the specialized services / environment to meet their care needs, up to an including end of life care. APL has a diverse inter-disciplinary team who collaborate with residents and families in their care and on meaningful goals and engagement. In accordance with the vision and values of the organization, APL’s approaches & principles include; Person Centered Care; Trauma Informed Care; Recovery in MH; and Community Engagement.QualificationsEducation & Experience
  • Completion of a recognized Social Service Worker or Social Service Diploma Program or an equivalent combination of education, training and experience.
Knowledge, Skills & Abilities
  • Ability to communicate effectively both verbally and in writing.
  • Ability to deal with others effectively.
  • Physical ability to carry out the duties of the position.
  • Ability to prioritize and organize work.
  • Ability to operate related equipment.
