- Receive inquiries about service, provide information on VHA’s services, and conduct intake assessments and completes documentation for service plans.
- Schedule Child and Family Support Workers (CFSW), conduct performance evaluations and supervise delivery of services to clients. Matches, schedules and supervises CFSW volunteers.
- Conduct Field Supervision on all clients, scheduling initial home visit assessments and conducting regular progress and follow-up visits and review and discuss care plans and make appropriate recommendations to all clients’ service.
- Facilitates team problem solving for service plans, Child & Family Support Workers (CFSWs) scheduling, supervision and other operational issues.
- Support service outreach and program promotion and liaise with contacts from other community agencies, and institutions and assist in developing strategies to meet local community needs.
- Bachelor of Social Work (BSW) required; Master of Social Work (MSW) preferred.
- Registered and in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
- Minimum of 3 years’ direct experience working with families with children facing mental health challenges.
- Demonstrated experience in family and mental health counselling, including crisis intervention and supportive counselling.
- Proven group facilitation skills, including program delivery and psychoeducational groups.
- Experience in client service coordination, case management, and staff supervision.
- Experience working collaboratively with Child and Family Support Workers, Volunteers, and Social Work students.
- Strong knowledge of community resources, social service systems, and best practices in family and mental health support.
- Ability to work effectively in a multidisciplinary team environment.
- Knowledge of the City, TTC routes, and the various components of the Agency preferred.
- Demonstrates excellent written and verbal communication skills to effectively engage with clients, families, and interdisciplinary teams.
- Computer skills in word processing and data entry.
- Valid driver’s license and use of a car on the job is a requirement.
