The role supports health promotion, illness/injury prevention, and care management in both clinical and community settings, in collaboration with individuals, families, and community partners.ESSENTIAL QUALIFICATIONS
- Active registration with ANBLPN and be in good standing
- Current BCLS certification
- Minimum2 years recent and direct experience (within last 5 years) in community-based nursing or a related area such as Addiction & Mental Health, Public Health, or Primary Health is required. A combination of relevant work experience and related education training will be considered.
- Commitment to complete area-specific training and certifications (i.e. Mental Health Interventions, Public Health Modules, Chronic Disease Management, immunization practices) as outlined during orientation and onboarding within a defined timeframe.
- Possess a valid driver's license and transportation, with the ability and willingness to travel within the region to support patient care, attend client appointment and participate in community-based activities as required.
- Possess a valid passport to support those in Area 2

- Participates in the development, monitoring of the plan of care, including maintaining document standards, in collaboration with the health care team, patients, and family members.
- Supervises and facilitates client activities to support health promotion, education, and self-management, including but not limited to mental health, chronic disease and wellness programs.
- Leads or supports group session to promote health knowledge, self-management and wellness across diverse populations and settings.
