Closing Date: September 18, 2025Position SummaryThe Rehab/CCC Coordinator serves as the clinical lead for education, training, and capacity building related to Rehabilitation (NRS, FIM) and Complex Continuing Care (CCRS/IRRS) reporting and quality initiatives.This role is based at the Bickle Centre and will focus on supporting Bickle programs, and may be required to provide cross coverage at other UHN Rehab and CCC sites. The Coordinator ensures that clinical staff and assessors are well-prepared to use CCRS, interRAI, NRS, and FIM tools, enabling accurate data collection, analysis, and improved patient care.Reporting to the Manager of Decision Support, the Coordinator will be part of a team of analysts and coordinators supporting UHN's complex care and rehab sites. The role works in close collaboration with clinical teams, site leadership, and corporate partners-including Finance, People & Culture, and Data & Analytics-to promote continuous learning, maintain data quality, and advance operational excellence.This position requires regular on-site presence at Bickle Centre to deliver education, provide hands-on support, and lead training initiatives.Key Responsibilities
  • Design, coordinate, and deliver training programs for interRAI, NRS, FIM, CCRS, and IRRS reporting tools for new and existing clinical staff.
  • Maintain certified trainer status for NRS and CCRS/IRRS, and manage monthly training schedules.
  • Develop, update, and maintain training resources, manuals, and online educational materials.
  • Provide individualized coaching to assessors and clinical teams to ensure accuracy, consistency, and confidence in assessment practices.
  • Assess training needs through data quality reviews and feedback; implement targeted education where required.
  • Lead the annual Professional Practice Day, providing updates on interRAI, NRS, FIM, CCRS/IRRS changes, and best practices.
  • Communicate and educate staff on CIHI methodology updates and ensure compliance across all reporting systems.
  • Facilitate clinician access to CIHI's online education resources.
  • Serve as the clinical expert for interRAI, NRS, FIM, CCRS, and IRRS reporting systems, troubleshooting workflow and system issues.
  • Collaborate with clinical teams, Decision Support, Finance, and Data & Analytics to ensure data integrity and effective use for operational decision-making.
  • Provide timely data quality feedback to reinforce training and support continuous improvement.
  • Oversee reconciliation, submission accuracy, and compliance with CIHI and MOHLTC standards.
  • Contribute to cross-site initiatives to standardize education and data quality practices.
  • Support patient placement and operational planning through education-driven best practices.
  • Actively engage in internal committees, CIHI forums, and working groups to align education strategies with broader organizational priorities.
  • Provide ongoing support for CCRS/NRS/FIM-related applications (e.g., Clarity, EPIC, or other vendor systems used for data collection and reporting). Responsibilities include:
  • Collaborating with clinical teams to gather requirements
  • Educating end users on how to effectively use data collection tools
  • Acting as a liaison between clinical teams and Digital/IT
  • Work collaboratively under Decision Support leadership with IT and vendors to coordinate system updates, testing, and troubleshooting.
  • Support change management by addressing help desk escalations and managing change requests related to clinical education and workflow adjustments.

  • Cross-site collaboration and occasional travel may be required.
  • Ongoing participation in CIHI educational forums and professional development is expected.
QualificationsQualifications
  • Master's degree in a regulated health profession - required
  • Current registration in good standing with a recognized regulatory college - required
  • Minimum 2-3 years of clinical experience in CCC, Rehabilitation, or Long-Term Care - required
  • Minimum 1 year of experience with interRAI, NRS, FIM, CCRS/IRRS assessment and training - preferred
  • Certified NRS and interRAI/CCRS Trainer - preferred
  • Strong expertise in adult education principles and facilitation
  • Proven experience developing educational materials and delivering clinical training programs
  • Proficient in Microsoft Office (Word, Excel, PowerPoint) and data analysis
  • Excellent communication, organizational, and interpersonal skills
